Principle of determining renin and aldosterone concentration and sample collecting

Renin is a proteolytic anzyme that affects angiotensin triggering a series of biochemical cascade reactions that lead to the generation of angiotensin II which is a strong vasoactive substance. Renin is dissolved from a nonactive prorenin generated and accumulated in granules of iuxtaglomerular renal cells. Angiotensin II affects directly smooth blood vessels causing vasoconstriction, and it also has an affect on increased aldosterone and catecholamine secretion from adrenal glands. In the physiological conditions the primary role of the renin-angiotensin-aldosterone system (RASS) is to regulate the blood pressure and salt and water matabolism. In the Department of Radiofarmacy and Radioimmunology of the Clinical Institute of Nuclear Medicine and Radiation Protection, Clinical Hospital Osijek, the blood samples are taken from hospitalized and ambulatory patients who underwent the captopril test and/or vein renal catetherization to determine renin and aldosterone concentration. Metods of collecting blood samples and determining renin and aldosterone activities are presented in the paper. The measurement of the plasma renin activity and the serum aldosterone concentration is performed by a radioimunological method (RIA) based on the competition between aldosterone and renin in blood sample and an antibody from a test tube. The plasma concentration renin is measured indirectly by determining angiotensin I concentrations that develope during in vitro incubation. We can conclude that this noninvasive method proved to be very useful for screening of different forms of renovascular hypertensions.
