Thyreoiditis subacuta de Quervain - a disease still harder to recognize
Besides usual clinical and diagnostic parameters, we were interested mainly in anamnestic data before diagnosing the disease, duration of the illness and its outcome. After the analysis, the resaults are: from 1980 until 2005 there were 434 patients with the diagnosis of subacute thyreoiditis (82 male and 342 female, mean age: 45 years, aged 14-79). Duration of the symptoms until diagnostic procedure lasted approximately 3.8 weeks (1-24 weeks). Average SE rate was 76/ mm/h. Palpatory solid, painful, rugged thyroid gland had 382 patients, and 52 were without these findings. Blocked scintigram had 387, unblocked 23, and in 24 patients we did not do the procedure. There were 15 patients with no pain, tolerable pain had 134, medium strong 237, intensive 48. 94 patients localized pain in the area of thyroid gland, 104 in neck and 236 mostly combined in neck and head (ears, back of the neck, lower jaw). There were 139 patients with no temperature, 295 with temperature and 89 of the patients knew exactly the duration of their temperature in days (average 16.9 days). Pain migration in another lobe had 121. Hyperthyreoid phase of the disease had 188 patients, and hypothyroid phase 94. 141 patients undertook ultrasound examination, with characteristic findings in 132 patients, without characteristic findings in 9 of them, without ultrasound examination 293. Fine needle biopsy was done in 379, was characteristic in 230, was not characteristic in 109, technical difficulties in 40, was not done in 55. There were 417 patients cured only with analgetics, and also with corticosteroids 9 of them, without therapy 17. Average duration of the symptoms lasted 9.9 weeks. Slightly more often it occurred from May until August. After relieving of the symptoms, 290 patients were followed for approximately 40.6 months. 24 patients became hypothyroid, 8 of them had positive anti-MSA or anti-TPO antibodies, 3 developed Graves's disease, and 3 Plummer disease.
