Damage associated malecular pattern ( DAMP ) molecules as mediators of systemic juvenile idiopathic arthritis.

Introduction: Systemic juvenile idiopathic arthritis (sJIA) is chronic childhood arthritis of unknown cause. Novel studies seem to point to deregulation of the innate immune system as the primary pathophysiological mechanism, thus making sJIA an auto-inflammatory disorder. Molecular signals that are able to stimulate the innate immune response and thus set off the inflammatory process could be broadly divided into exogenous, or pathogen associated molecular patterns (PAMPs), and endogenous, or damage-associated molecular pattern molecules (DAMPs) also known as alarmins or endokines. Alarmins are a group of structurally diverse multifunctional proteins that are rapidly released during tissue damage (including infection), and that have mobilizing and activating effects on cells involved in host defense and tissue repair. The family members include high mobility group box 1 (HMGB-1), heat- shock proteins (HSP), monosodium urate crystals, altered matrix proteins (such as hyaluronan), and calcium binding proteins of the S100 family
