Sarcoidosis and rheumatoid arthritis – a rare association
Sarcoidosis is a multisystem granulomatous disease, commonly involving skin. Rheumathoid arthritis is a systemic disease with arthritis as primary manifestation but skin manifestations can also be present. There are similarities between these two diseases, but their association is very rare. Sarcoidosis is an immunomediated disease characterized by an increased Th1 immune response. Also there is evidence that the rheumatoid arthritis is triggered by Th1 lymphocyte activation. The etiology of both diseases remains unknown. We report a rare coexistence of these two diseases in 63-vear-old man. We presents this patient who complained of fatigue, arthralgia affecting almost all joints symmetrically and developed small pink papules on the face, neck and fingers. Laboratory, radiograph, chest CT scan histology investigation revealed presence of skin and pulmonal sarcoidosis and rheumatoid arthritis. The patient has had a good response to corticosteroid therapy and is still followed up.
