Tinea incognito due to Microsporum canis infection
Tinea incognito is a steroid modified dermatophyte infection with atypical clinical presentation. It is caused by prolonged use of steroids that are often prescribed as a result of incorrect diagnosis. We report a case of 76-year-old housewife with multiple, erythematous, scaly and itchy patches over the face and neck along with alopecic, erythematous, scaly patches in the occipital region of the scalp. Direct microscopy of scraping specimens was positive for fungal hyphae. Woodlight inspection of hair scalp revealed green fluorescence. Fungal culture on Sabourad's agar grew Microsporum canis. Four weeks oral treatment with terbinafine and 8 weeks topical terbinafine therapy resulted in complete clinical and mycological cure.
