Bone Tumor Incidence at the Department of Orthopaedic Surgery Zagreb University School of Medicine from 1981 to 2002

AIM: To determine the real bone tumor incidence in patients operated at the Department of Orthopaedic Surgery Zagreb University School of Medicine in the period from 1981 to 2002. METHODS: The retrospective descriptive study. All patients with tumors or tumor like lesions of locomotor's system were investigated. Clinical findings, radiological and laboratory tests and finally pathohistological diagnose after biopsy were controlled for each patient. In total 50, 729 operative procedures performed at our Department in 21 years, 2127 patients were operated due to diagnosis of locomotor's tumor or tumor like lesion. Mean age of our patients was 30.8 years (SD=18.5) ; 8% of patients were under 10, and 33% of patients were between the age of 11 to 20. Sex distribution was roughly equal (M:F = 1102:1025 = 51.8%:48.2%). RESULTS: Tumoral problems take 4.2% of all operative procedures at our Orthopaedic Department. According to histological findings, the first three most common groups are: cartilage forming tumors (754 pts ; 35.4%), bone forming tumors (316 pts ; 14.9%) and tumor-like lesions (298 pts ; 14, 0%). Top 10 of the most common operated tumors and tumor like lesions according to diagnosis were: osteochondroma (487 pts ; 22, 9%), solitary (juvenile) bone cyst (189 pts ; 8.9%), soft tissue tumors (163 pts ; 7.7%), enchondroma (158 ; 7, 4%), osteosarcoma (137 ; 6, 4%), osteoid-osteoma (116 pts ; 5.5%), giant cell tumor of bone (90 pts ; 4.2%), metastatic bone tumors (82 pts ; 3.9%), Ewing sarcoma (78 pts ; 3.7%), and lipoma (70 ; 3, 3%). Tumors were mostly localized in distal (13.6%) and proximal femur (9.8%), and in proximal tibia (8.8%) ; region of knee joint is the most common localization of the bone tumor. CONCLUSION: Yearly, about 100 new patients with oncological - tumoral findings were operatively treated at the Department of Orthopaedic Surgery in Zagreb. Bone tumors and tumor like lesions with general incidence of about 4, 2 %, and for its complexity take important place in modern orthopaedics, and should be taken in consideration in planning of hospital beds.
