Transverse myelitis - differential diagnosis and treatment strategies
Transverse myelitis (TM) is a rare clinical syndrome, characterized by focal inflammation, demyelination, and axonal damage within the spinal cord as well as clinical manifestations that are due to resultant neural dysfunction of motor, sensory or autonomic pathways within and passing through the inflamed area. There is often a clearly defined rostral border of sensory dysfunction and evidence of acute inflammation demonstrated by a spinal MRI and cerebrospinal fluid (CSF) analysis. TM may exist as part of a multifocal central nervous system (CNS) disease (e.g.multiple sclerosis), multisystemic connective tissue disease (e.g. systemic lupus erythematosus, Sjögren's syndrome), or as an isolated, idiopathic entity. The authors summarize recent classification and diagnostic schemes, with extended description of differential diagnosis of transverse myelitis, including compressive, vascular, inflammatory etiology, etc. Additionally, special review has been dedicated to the current concepts of treatment strategies.
