Evolving trends in elective splenectomy – five years single surgeon experience
Over the past 5 years single surgeon performed 20 elective splenectomies (18 laparoscopic, 2 open) and one laparoscopic spleen cystectomy. Patients were operated due to NHL (8 pts), ITP (5 pts), splenic cyst (4 pts), AIHA (1 pt), 1 hypersplenism due to Leishmaniasis (1 pt), CLL (1 pt), and spherocytosis (1 pt). The surgeon used three (13 pts) or four trocars (5 pts) approach. There was one conversion, no intraoperative or postoperative complications. Mean spleen size was 19.6 cm (largest diameter 28 cm). The vascular structure was controlled safely using a vascular Endo stapler, clips or Ligasure. Mean operating time was 170 min, hospital stay 5 days, 8 pts required blood transfusion and the mean blood loss was 410 ml. 19 pts remain in remission, one died 4 years after surgery and one have a recurrence of ITP. Laparoscopic splenectomy is safe and feasible but splenic size larger then 15 cm or 1 kg requires caution and common sense.
