Oral Health Related Quality of Life (OHRQoL) in mandibular complete denture wearers with different dental status in the maxilla

The aim: The aim was to assess the oral health related quality of life (OHRQoL) in mandibular complete denture wearers (CDWs) with severely resorbed residual alveolar ridges dependent on the dental status in the maxilla. Materials and methods: A total of 40 CDWs were selected and divided into 3 groups: 18 of them had complete denture (CD) in the maxilla, 10 of them had maxillary removable partial denture (RPD) and 12 of them had natural teeth and/or fixed partial denture (NT/FPD). Each subject completed the OHIP49-CRO questionnaire. Statistical analysis included descriptive statistics and one-way ANOVA with Scheffe post-hoc. Results: The mean age of mandibular complete denture wearing was 8.4 years. Except for the questions considering Psychological discomfort, subjects with NT/FPD in the maxilla had the highest scores of all other six domains of the OHIP, as well as of the OHIP summary score. The NT/FPD group had significantly higher scores than the CD and the RPD group (p<0.01) for the questions considering functional limitation and physical disability In the domain of psychological discomfort, psychological disability, social disability, handicap and the OHIP summary score, subjects with maxillary CDs had significantly lower scores than those with RPDs and/or NT/FPDs (p<0.01). Conclusions: The results indicate that the worst OHRQoL is registered in mandibular CDWs with antagonistic natural teeth or FPDs, while the best OHRQoL is registered in mandibular CDWs with maxillary CDs as antagonistic teeth. This could be attributed to the different extent of stress forces exerted to the denture bearing area, and/or to the different status in the maxilla that patients compare their mandibular CDs with.
