Patients with mandibular complete dentures supported by mini dental implants: A 1.5-year longitudinal study considering oral health related quality of life

Aims: To evaluate changes of the oral health related quality of life (OHRQoL) in patients with mandibular complete dentures (CDs) after flapless insertion of mini dental implants (MDI) and immediate loading. Materials and Methods: In 28 mandibular CD wearers with severely resorbed residual alveolar ridges, flapless implant surgery was made and four MDIs (IMTEC, MDI Sendax) (1.8 or 2.4 mm diameter, length varying from 10-18 mm) were placed in immediate function in the frontal region of the mandible to support CDs by ball-O-ring attachment. The OHIP49-CRO was obtained from each patient twice: one-month prior the MDI placement and after the 1-1.5 year period of function. Panoramic radiographs were obtained immediately after the MDI insertion and after the 1-1.5 years. Patients also completed the question using the 5-point Likert scale (1=not satisfied at all, 5=completely satisfied) considering their satisfaction prior and a year after the MDIs insertion. Results: From a total of 112 implants, only one implant was lost during the first 3 month after insertion, but CD was successfully supported by the remaining 3 MDIs. At the one-year stage the bone level showed no significant changes around the implants (p>0.05). The OHRQoL was extremely improved considering the OHIP summary score and all seven domains (Functional limitation, Physical pain, Physical disability, Psychological disability, Social disability, and Handicap) (p<0.001). The effect size according to Cohen was high (5.17). The OHIP summary score changed from 81.79 to 8.14. The satisfaction significantly increased from 2.07 to 4.64 (p<0.01). Conclusion: The use of 4 MDIs in the frontal region to support mandibular CDs in patients with resorbed and thin residual alveolar ridges demonstrated significant increase in OHRQoL, overall satisfaction, and the high effect size with good preservation of bone level at the 1.5 year stage.
