Trauma of maxilofacial region – MDCT evaluation

Nowadays, the most prevalent causes of maxillofacial injuries are motor vehicles, physical violence and falls. Because of the complex anatomy in maxillofacial region and the superimpositions of structures, the radiographic interpretation of fractures of this region is difficult. So, after conventional radiography, these patients are undergone to MDCT scanning. In the Department of diagnostic and interventional radiology University Hospital Sestre Milosrdnice MDCT examination is performed by 16-slice CT scanner. “ Low-dose“ protocol is used for patients with isolated facial trauma. For politraumatized patients we use protocol „ Head Spiral“ with thin slice 1 mm, image interval 0.5, pitch 0.55. In post-processing raw data are used for multiplanar reconstructions in high resolution and standard mode, and for 3D- Volume Rendering at workstation Leonardo Syngo 240. This provides a suitable image detail to detect bony fractures and changes in the soft tissues. Due to the variety of fracture patterns in this region, an exact and correct three-dimensional location of the fracture line is essential for the satisfactory surgical management in this pathology. During the last year we had 37 patients with maxillofacial injuries in our Department. We shall present several of them who are undergone to MDCT scanning.Conclusion: MDCT is the most efficient imaging modality for visualisation of maxillofacial injuries because this procedure is available, noninvasive (comfortable for patient), takes short time and of the reasonable cost. Radiologist becomes essential member in interdisciplinary approach in establishing diagnosis and further plan of treatment of these injuries. He has the main role in evaluation of maxillofacial injuries with his great knowledge and experience.
