Interstitial lung disease as the commencing manifestation of connective tissue disorders-clinical characteristics

Connective tissue diseases (CTD) frequently affect the respiratory tract. The interstitial lung disease (ILD) as the commencing manifestation of CTD is recognized more often than earlier. The aim of our study is to analyze the clinical characteristics of patient who were evaluated for ILD of unknown cause, and finely were diagnosed as CTD. We report the clinical characteristics of 24 patients who met the above criteria. There were 24 pts, 14 female and 8 males, age from 37-75, average 57.3 years. Following the diagnostic procedures (data available) the final diagnosis was reached: 5 pts had MCTD, 5 scleroderma, 4 RA, 4 SLE, 2 polymyositis, and 4 unclassified CTD. The frequency of respiratory symptoms and signs was: dyspnoea 75%, cough 66.6%, haemoptysis 4.1% and crackles 83.3%. Systemic symptoms appeared: fatigue 41%, fever 29.1%, weight loss 8.3%, pallor in 4.1%, and sweating in 12.5% of pts. The joints were frequently affected ; arthralgias were found in 45.8%, swelling of the hands/fingers in 20.8%, and finger clubbing in 4.1% of pts. Other findings were: Raynaud’ s syndrome in 33.3%, skin rash 8.2%, facial skin tightness 4.1% and DVT in 4.1% of pts. One patient only had no other signs and symptoms but respiratory. The pts were adequately treated, and followed up for 0-7 years. Two patients died, four worsened, but majority is stable or has improved. Although the presenting symptoms were respiratory, the majority of these pts also had systemic, joint or other extrapulmonary signs and symptoms. The search for extrapulmonary manifestations prompts the exact diagnosis and consecutive adequate management.
