Surgical treatment of the clavicular non-union with bone graft

Fractures of the clavicle are common injuries constituting 4- 16% of all fractures. Clavicular fracture traditionally have been treated nonoperativelly. However, most recent report suggest that nonunion after nonoperative treatment of displaced clavicular fracture is much higher than previously thought . Nonuninon of the clavicle can cause pain and shoulder disability, urging for surgical treatment. Several surgical techniques for the treatment of the clavicular nonuninios has been reported. The aim of this article is to asses the results of the operative treatment of the clavicular nonunion using autogenous iliac bone graft. We also wanted to determine whether primary way of treating clavicular fracture (nonoperative or operative) will influence postoperative results. We have analysed retrospectively 16 patients. Nine patients were initialy treated nonoperatively, seven operatively. All patients were treated by open reduction and internal fixation with plates and screws. Autogenous bone graft from iliac bone was used in all patients. Average follow up was 58 months (range 36 to 120). Bone healing was achived in all patients. Constant score was an average 91 (range 82 to 100). Primary nonoperitively or surgical treatment of clavicular fracture did not influence on the postoperative results. Surgical treatment of the clavicular nonunion with plate and bone graft should be used as standard technique for nonunion of the clavicule.
