Anesthesia risk and preoperative evaluation of the geriatric patient
Because multisystem dysfunction is often present in older patients, they present a unique challenge in preoperative evaluation and postoperative management. Increasingly older and sicker patients are receiving more complicated operations. In this article we review physiologic changes of normal aging and its implications in anesthesia practice. Preoperatively anesthesiologist must perform detailed and physical examination with special focus on cardiovascular, pulmonary, renal and cerebrovascular risk factors. Sometimes adequate assesment can be accomplished from anamnestic information or by performing simple clinical tests. In other instances a more detailed testing or multidisciplinary approach is required. A carefully prepared preoperative evaluation will enable the physician to modify and plan management to decrease postoperative morbidity and mortality in the older patient.
