The Influence of Histological Type of Tumor on Survival Prognosis in Patients with Ovarian Cancer

Aim: Study was undertaken to examine, on bioptical material, connection of histological type of tumor and prognosis in patients with ovarian carcinoma. Methods: In this study, the surgical material of 80 patients operated in Department of Gynecology, Clinical Hospital Split in period 1989.-1994. with surface epithelial stromal carcinoma of the ovary was examined. Formalin fixed and paraffin embedded tissue was examined using standard hemalaun-eosin method. Histological type of tumor was defined by who classification, modificated by RE Scully. Results: Analyzing the influence of histological type of tumor on survival prognosis we found statistically significant connection between lower rate of survival and serous histological type (p=0, 0107), clinical stage (p=0, 0001). Discussion: Malkasian and collaborators have valued the prognostic significance of histological type of tumor on large number of women with ovarian cancer. They concluded that lower rate of survival found in patients with serous and nondifferentiated tumors is loosing its meaning if we make correction by stadium, because serous and nondifferentiated tumors are more often found in advanced clinical stage then other histological types. In this study, almost ¾ (73%) serous carcinoma are found in advanced clinical stages, compared to mucinous tumors, where we found reverse situation: about ¾ (71.4%) are found in early, and ¼ (28, 6%) in late clinical stage, so the difference in survival should be ascribed to clinical stage, and not the histological type itself. Conclusion: Serous histological type of ovarian cancer is connected with worse prognosis. Patients with serous histological type of tumor are more often in advanced clinical stage than patients with other histological types of ovarian tumors.
