Epidemiological data on the clinical status of myocardial infarction patients'

In our study we included 10, 579 cases of acute myocardial infarction (AMI) hospitalised in five Zagreb hospitals during a ten-year period (from 1984-1993). We investigated the clinical status of myocardial infarction patients (pts) with regard to hospital survival, sex, age and reinfarction. Of the total number of patients 3, 623 (34%) were women. 2, 812 pts died in hospital, so the hospital case fatality was 27 %. 1, 594 men and 1, 218 women died in hospital, so the hospital case fatality was 23% for men and 34% for women. We found out that, when taking into consideration age groups and case fatality, there were more fatal cases in the older age groups. In the age group >74 there were 2, 497 cases of AMI and 1, 113 out of them died, so the case fatality rate for this age group was 45%. 3, 076 (29%) of the AMI pts had a congestive heart failure (CHF), 396 (4%) were in a state of cardiogenic shock, and cardiac arrest was recorded in 163 (2%). Data on clinical status was unknown in 697 (7%) pts. CHF was more often observed in women than men ; 1.417 (39%) women and 1, 659 (24%) men had CHF. Of those who survived the attack - 2, 405 women and 5, 362 men - 822 (34%)women and 990 (18%) med had CHF. Women who died of AMI (1, 218) had CHF in 595 cases (49%), and men who died of AMI (1, 594) had CHF in 669 (42%). CHF and cardiogenic shock were observed more often in older pts ; in the age group >74 these were recorded in 54% pts. Death following cardiogenic shock and cardiac arrest were recorded in 16% of women and 18% of men. Reinfarction was observed in 635 (18%) women of whom 51% had CHF and in 1, 547 (22%) men out of whom 33% had CHF. Women for whom that was the first attack had CHF in 37% and men in 22% of the cases. Conclusion: The prognosis after AMI was poorer in women: hospital case fatality was higher and CHF was more often observed. CHF was more often recorded in older age groups and in those with reinfarction, both in women and men.
