Influence of atrial fibrillation on the outcome of patients with AV block and DDD pacing
The impact of atrial fibrillation (AF) on clinical outcome of patients (pts) with chronic AV block and DDD pacing has not been completely explored. The objective of this study was to determine the occurrence of AF in these pts and to evaluate its influence on the mortality and heart failure. The occurrence and prognostic impact of AF were studied in 132 pts with chronic AV block who received DDD pacing. Clinical variables and outcome of pts were compared regarding to the development of permanent AF over period of at least 1 year. In the group of 132 pts with chronic AV block and DDD pacing, 13 (9.8%) pts have later developed permanent AF and 119 have not. Pts with AF were older and had significantly smaller P wave amplitude at the baseline evaluation (p<0.01). During follow-up, 31% of the pts with AF died compared with 8% of those without AF. 36% of the pts with AF and 6% of the pts without AF developed heart failure (p<0.01). In pts with chronic AV block and DDD pacing, development of the premanent AF increases the risk of death and of heart failure.
