Sažetak
Background. B-type natriuretic peptide and NT- proBNP are serum biomarkers for heart failure that are released into the circulation in response to ventricular dilatation and pressure overload. These peptides have been shown to be very powerful prognostic markers and has potential role for the monitoring of therapy. As prognostic markers, it seems that serial testing carries important information. The aim was to assess time-course of BNP and NT-pro BNP in myocardial infarction. Methods. Samples from 14 hospitalized patients with myocardial infarction were investigated. BNP (ABBOTT, AXSYM) and NT-proBNP (ROCHE, MODULAR E170) were measured in serum at admission and after 2, 3, 7 and 14 days. Clinical history, electrocardiography and echocardiography was done within 24 h of admission. Results. NT- proBNP values were elevated in all samples. For most patients, highest values were obtained between second and third day, except for two patients who were admitted with developed infarction. All patients had highest values for BNP between second and third day, but values on admission were elevated only in five of them, while one patient had peak at second and seventh day. Conclusions. Mentioned differences are probably result of distinction between plasma half-life of BNP and NT-proBNP. Because NT-proBNP has longer half-life, it’ s blood level remains elevated longer than BNP level, which results in it’ s higher diagnostic sensitivity to detect early stage of LV- dysfunction. In opposite, it seems that BNP has faster rise and decline and probably can be suitable to detect reinfarction.
