Relationship between the extent of autonomic neuropathy and venous reactivity in patients on hemodialysis

Fifteen patients (7 females, 8 males) aged 36-73 years were studied to determine whether there is a relationship between the extent of autonomic neuropathy and venous reactivity in uremics on chronic hemodialysis. Results of the study showed abnormal Valsalva's maneuver in 13, heart rate variation on deep breath in 9, heart rate response to standing in all, and a significant blood pressure decrease in the standing position in 8 patients. A prolonged QTc interval was recorded in 8 patients. Venous tone reactivity was measured by the "venoconstriction" test: intraveous pressure changes were identified by injection of 2 mcg of noradrenaline and expressed in venoconstrictive units (VCU). The venous tone response ranged from 895 to 5394 VCU. There was a significant correlation between the autonomic function score and venous reactivity to catecholamines (r=0.524, p=0.045). It is concluded that there is a direct relationship between the extent of autonomic neuropathy and venous reactivity in uremic patients on hemodialysis.
