Ortopedsko zbrinjavanje djece i odraslih sa Downovim sindromom

The aim of this study is to emphasize the spectrum of the most common musculoskeletal problems in children and adults with Down s syndrome.Orthopaedic management should be focused on spine, hip and knee problems. A survey was performed of 36 persons (24 males) ascertained as having Down s syndromein three institutions in Zagreb, using a special questionarre for muskuloskeletal problems, In addition, a medical records since 2000 in our institution were reviewed.Five individuals (all males)were surgically treated due to hip or knee problems.We found that out of 41 patients examined with Down s syndrome nine (22%)patients had 13 significant muskuloskeletal problems (four patients had bilateral problems).Six patients (three bilateral) had patellofemoral instabillity ; two patients (one bilateral) had hip problems and one patient had 30 degree lumbar scoliosis. Clinical and radiological results in the surgically treated patients, with minimum one year follow up, showed very good functional outcome for all patients.One patient was surgically treated twice on the same knee joint, due to the persistence of patellofemoral instability.We conclude that more than 20% of the Down s syndrome population has significant muskuloskeletal problems that with precise indications and individually tailored surgical procedures could be functionally improved without complications.
