Sažetak
The first report of HFRS in Croatia was by Radošević and Mohaček, who described four cases of nephropathia epidemica Myhrman-Zetterholm. In 1995, we recorded the largest HFRS in Croatia to date. Among 203 suspected HFRS patients, we confirmed approximately 42% of them by conventional ELISA IgM and IgG tests and Hantan and Puumala antigens. During the outbreak, we were looking for immunophenotypic changes in the main lymphocyte populations and activation markers in 22 HFRS patients. We used EIA-test (Quantikine, R&D Systems Inc., UK) for the detection of IL-2, IL-2sR-alpha, IL-6 and IL-6sR in sera of 41 HFRS patients. PCR analysis and recombinant ELISA test on 36 serum samples is in process. Our aim was to correlate clinical and biochemical data with the immune response, and type of hantaviruses which caused the HFRS and to look for their role in the pathogenesis and prognosis of HFRS. We found elevated levels of IL-2sR-alpha, IL-6 and IL-6sR in patients with HFRS in comparison to healthy controls. IL-6 showed negative correlation with the day after the HFRS symptoms appearance. IL-2sR-alpha positevely correlated with the CD25, an early activation marker. Patients had elevated levels of aspartate (AST), alanine (ALT), alpha-2-globulin, creatinine, urea, sedimentation rate (SR), white bolld count (WBC) and gamma-g-glutamyl transferase (gamma-GT). In 15 patients we found positive correlation among IL-2sR-alpha and AST (r=0.85, p=0.001), ALT (r=0.54, p=0.04) and gamma-GT (r=0.54, p=0.04), than for CD25 and AST (r=0.68, p=0.005). Analysis of other results is in process.
