Cardiac disease as an important risk factor for the occurrence of ischemic stroke

The aim of this study was to evaluate the contribution of cardiac diseases to the development of ischemic stroke. During 30 consecutive months, 1, 056 patients (64.78% of the total number) were hospitalized for ischemic stroke at the Department of Neurology, Osijek University Hospi-tal, in Osijek, Croatia. In addition to cerebrovascular dise-ase, 242 of them had a cardiac disease. These patients were divided into two groups: group 1 consisted of patients with functional cardiac diseases (angina pain syndrome, atrial and/or ventricular fibrillation, supraventricular and ventricular extrasystole, conduction block or disturbed repolarization), and group 2 included patients with acute or subacute myocardial infarction and its sequels. All pa-tients were examined by a cardiologist and underwent electrocardiography. Most of the stroke cases were diagno-sed by computed axial tomography of the brain. Patients with functional cardiac diseases (69% ) outnumbered tho-se with morphologic cardiac diseases. Atrial fibrillation was found in about 8% of the total number of patients. A peculiar correlation was recorded between the duration of cardiac disease and occurrence of stroke. Patients who had suffered from cardiac disease for up to 5 years had a signi-ficantly greater prevalence of stroke than those with cardi-ac disease duration of 6-10 or >_ 11 years (p<0.01 both). There was no significant difference in stroke occurrence between the groups of patients with longer duration of car-diac disease. It appears that cardiac disease is a major risk factor in the development of ischemic stroke. Functional disorders contributed significantly to cerebrovascular di-sease. It was concluded that these diseases should be early detected and treated in patients with compromised brain circulation in order to reduce the risk of ischemic stroke.
