Lletz versus cone: our experience
Retrospective examination from 1 December 1994 to 31 January 1997 in 105 women with LLETZ and 97 with cold knife conisation. In LLETZ group more were younger women (20-29 yrs) whereas in coned more were 30-40 yrs. The severe dysplasia in LLETZ specimen was found in 31 (29.5%). In age group 24-29 yrs there were 15 with severe dysplasia (14.3%). The same diagnosis in cold knife conisation was present in 46 (58.2%) and specially increases in age group 30-40 (37 of 79 or 46.8%). In younger women of conisation group there were 9 severe dysplasias of 11.4%. According to our experience LLETZ is suitable in younger women, especially nulliparous who need thorough preservation of procreation. Although there was a slight fall in severe dysplasia in younger age groups in conisation specimens, general concern about high grade dysplasia has to be maintained because of its serious neoplastic potential.
