Renal tubular defect in patients with idiopathic membrane nephropathy

Changes in renal tubuli in patients with idiopathic membrane nephropathy(IMN)have not been sufficiently investigated, although it is known that essential pathomorphological changes occur in tubuli in addition to their functional lesion in some other forms of glomerulopathies.The aim of our study was to investigate tubular function with regard to the number of involved tubuli and the quantity of immune complexes in them, and the activity of immunosupressives and cytostatics upon these changes. The 38 patients with IMN were treated.They were devided into two groups.In the first group were patients with IMN who were on symptomatic therapy, and in the second group patients were treated by corticosteroids and cytostatics. Laboratory and histologic investigations of renal tubuli showed rather great damage of their function.The degree of histologic damage depended upon the quantity of immune complexes in tubular walls, and the degree of functional damage depends upon the percentage of tubuli which contained immune complexes.The recovery of tubular function was considerably more rapid in patients treated by immunosupressive therapy. In conclusion, renal tubular function in patients with IMN depends considerably upon the quantity of immune complexes in tubular walls, and convalescence is more rapid in patients on immunosupressive therapy.
