Residual ridge resorption in removable denture wearers

Objectives: Wearing of different types of removable dentures may have adverse effects on the health of denture-supporting tissues. The purpose of the study was to determine the changes in mandibular bone mineral density in complete and removable partial denture wearers, to determine whether the mandibular bone mineral density is correlated with thickness and structure of the mandibular inferior cortex and to compare mandibular resorptive changes with cervical vertebral changes in edentulous elderly patients. Methods: A total of 136 randomly selected complete and removable partial denture wearers, 40 men and 96 women, participated in the study. The following parameters were evaluated on panoramic radiographs: cortical width at gonion, at antegonion, and below mental foramen, the appearance of the cortex of the lower border of the mandible due to resorptive changes and mandibular bone mineral density. The morphology of the bodies of the cervical vertebrae and measurements of linear vertical resorption in the frontal region of the jaws were analysed in cephalometric observations. Results: The results revealed statistically significant difference in cortical thickness and bone mineral density between complete and removable partial denture wearers (p<0.001) with the values higher in removable partial denture wearers. The severity of structural changes in the mandibular cortex was significantly related to mandibular bone mineral density values (p<0.01). The cervical vertebrae exhibited significant decrease in the height and width, and residual alveolar ridges exhibited significant decrease in the height over the 5-year period of wearing complete dentures (p<0.01). Conclusions: Removable partial denture wearers have higher mandibular bone mineral density and thicker mandibular inferior cortex than complete denture wearers. Patients having lower bone mineral density values in the mandible have much more porous cortical layer of the inferior border of the mandible. The size of the cervical vertebre and the residual ridges height change in elderly patients.
