Eosinophilic granuloma of the thyroid gland: Case report

Introduction: Eosinophilic granuloma is pathological and clinical entity characterized by abnormal proliferation of Langerhan's cells. Usually, it is mild and localized disease which most commonly occurs within bone tissue of children aged 5 to 10. It makes up 60-80% of all cases of Langerhan's cell histiocytosis. Hand-Schüller-Christian disease represents the intermediate, more agressive form characterized by multifocal, chronic involvement and Letterer-Siwe disease is the fulminant disseminated type. Here we report unusual presence of eosinophilic granuloma within thyroid gland tissue. Material and Methods: M.F., 47, was hospitalized because of large multinodal euthyroic goiter. Lobectomy of right thyroid lobe was performed, followed by histopathological analysis. Results: 20 histopathologically benign nodes which differed only in diameter were found. Surprisingly, among them, one 2 cm large node appeared different: it was gelatinous, dark red with numerous small white flecks within it. Histopathology reported predomination of histiocytes with large nuclei. Diagnosis was confirmed by immunohistochemical staining for S-100 protein, which was clearly positive. Additionally, intracellular Birbeck granules were also demonstrated by electron microscopy. Conclusions: This work reports very unusual location of eosinophilic granuloma within thyroid gland together with atypical age and sex of the patient. Thus, it adds new information to current knowledge about this disease and could help to understand its still mysterious pathogenesis.
