Implantation of Wagner revision endoprosthesis in treatment of primary bone tumors in proximal femoral part

Aim: Implantation of different models special tumor endoprosthesis is standard surgical procedure after resection of bone tumor in the region of proximal femur. Implantation of endoprosthesis for revision model Wagner in primary treatment of bone tumor is one of possibilities. The aim of this work is to report results of implantation of Wagner revision endoprosthesis in treatment of primary bone tumors in proximal femoral part. Material and Methods: From January 1998 to December 2003 implantation of 16 endoprosthesis model Wagner was preformed in patients with bone tumors at the Department of Orthopedic Surgery Zagreb University School of Medicine. We treated five patients with Ewing sarcoma, two patients with osteosarcoma, chondrosarcoma, metastasis of breast carcinoma, and metastasis of adenocarcinoma, and one patient with desmoplastic fibroma, plasmocytoma, and giant cell bone tumor. The average follow-up of patients was 30, 3 months (range, 4 to 72). The mean age of the patients was 29, 8± 18, 0 years ; the youngest patient was a 7-year girl with Ewing sarcoma, and the oldest patient was 58 years female with metastasis of breast carcinoma. Results: At the last review, two patients had died due to the tumor. On control examination stability of Wagner hip endoprosthesis was registered in 14 patients. In four young patients subluxation of the femoral head was registered. In three patients acetabuloplasty with massive homologous bone transplant was performed after 1, 3 and 4 years. In one patient aorto-femoral bypass had to be performed 2 days after resection "en bloc" tumor. The mean resection "en bloc" was 12, 8 cm (range, 6 to 22). Conclusion: Reconstruction with a modular prosthesis after resection of a tumor gives good functional results in most of the cases. If urgent reconstruction of proximal femoral part was indicated, good results were achieved with implantation of Wagner endoprosthesis. Stability of endoprosthesis was excellent even in the cases with relatively small intramedular part of Wagner stem fixed in cancellous bone of distal femoral part.
