Relationship between serum antibody profiles against Helicobacter pylori and gastroduodenal disease outcome

INTRODUCTION: Infection with Helicobacter pylori is associated with development of a variety of gastroduodenal diseases. Although Helicobacter pylori is common, the development of peptic ulcer disease and gastric carcinoma strikes only a small percentage of carriers.We evaluated the antibodies profiles to Helicobacter pylori virulence proteins to determine a relation between host response and outcome of infection. AIMS & METHODS: The study was conducted on 207 patients undergoing upper endoscopy. The Western blot method was applied to determine the presence of antibodies to p 120 (CagA), p 95 (VacA), p 67 (FSH), p 66 (UreB), p 57 (HSP homolog), p 54 (flagelin), p 33, p 30 (OMP), 29 (UreA), p 26, p19, and p17 proteins of Helicobacter pylori bacteria in the serums of Helicobacter pylori positive patients. The patients were assigned to gropus according to the diagnosis: 46 patients had gastric ulcer, 68 duodenal ulcer, 30 carcinoma, 12 gastric and duodenal ulcer and 51 were diagnosed as non 2 test (STATISTICA ulcer dispepsia. Statistical analysis was performed using for Windows, Release 5.5 H i SPSS for Windows, Release 7.5). RESULTS: P95 (VacA) seropositivity was more freequent in duodenal ulcers and non ulcer dispepsia and less freequent in carcinoma than in all other Helicobacter pylori positive patients (2 14.580, df 4, p <0.006). The p 120 (CagA), p 67 (FSH), p 66 (UreB), p 57 (HSP homolog), p 54 (flagelin), p 33, p 30 (OMP), 29 (UreA), p 26, p19, and p17 response was not seen more frequently in Helicobacter pylori patients with peptic ulcer, carcinoma or non ulcer dyspepsia. CONCLUSION: Host response to p95 (VacA) antigen could indicate patients with higher risk for duodenal ulcer and non ulcer dyspepsia outcome but lower risk for carcinoma outcome.
