Bariatric surgery in Croatia

The expansion of bariatric surgery seen in America, Australia and Europe has also affected Croatia at the beginning of the 3rd millennium. Minimally invasive surgery is performed in Croatia since 1992/93 and contributed largely to this expansion. Mason’s vertical gastric banding (VGB) represented for a decade the only open surgical procedure that had an adequate patient follow-up. Open gastric bypass procedures are performed since 2004. The first LAGB was performed in May 2005 and the first sleeve resection with duodenal switch was done in 2006. 18 patients were treated with LAGB ; the average initial body weight was 150.64 kg (range 127-204 kg) and initial BMI was 50.81 kg/m2. Weight, BMI, Excess Weight Loss (EWL) and complications were monitored. 40 patients had VGB. Roux-en-Y gastric bypass was performed in 7 patients. The average EWL in the LAGB group was 18.71, 25.06, 33.80, 40.17 and 44.41 % at 1, 3, 6, 9 and 12 months, respectively. The average EWL at 1 year for VGB is 60%, and was retained at 50% in 90% of patients after 4 years. The average EWL for Roux-en-Y gastric bypass is 58%. Several surgical procedures for morbid obesity have been performed in Croatia since 1970, mostly small intestine bypass surgeries. Even so, no clear prospective study of bariatric surgery has been done. The first important study on bariatric surgery was published in 1998. Bariatric surgery is performed in four centers today.We hope that will demonstrate larger series and better results in future world meetings.
