Hypertension as risk factor for perioperative blood transfusion in patient with total knee

Total knee arthroplasty (TKA) is an operative procedure associated with major perioperative bleeding. In addition to large quantity of crystalloid and colloid solutions, many patients receive perioperative allogenic and/or autologous blood transfusions. The application of allogenic blood transfusion is associated with many risks and complications. we examined the effects of clinical factors (hypertension, age, sex, overweight and type of anesthesia) upon increased utilization og blood transfusion. The aim was to examine the effects of hypertension, alone and in combination with other factors, upon the increased utilization of blood transfusion in TKA patients. In the period from 19999 to 2004, a retrospective study was conducted in the University Hospital of Traumatology Zagreb. The analysis comprised 78 patients submitted to the total knee arthrosplasty. There were 50 female and 28 male patients, of whom 47 were overweight. 69 patients were over 60 years of age and 9 patients under 60 years of age. Regional anesthesia was applied in 67 and general in 11 patients. The following clinical parameters were assessed:age, sex, history of hypertension and other disorders and body weight as well as their relationship to the increased blood loss and usage of blood transfusion. Out of 25 patients who received 1000-1500 ml of blood, 21 had arterial hypertension, 17 were overweight, 23 were over 60 years of age. Out of 13 patients who received 1500-2000 ml of blood, 11 had arterial hypertension, 8 were overweight and 10 were over 60yrs of age. More than 2000 ml og blood were transfused in 9 patients, of whom 7 had arterial hypertension, 4 were overweight and 7 were over 60 yrs of age. The analysis showed that hypertension, overweight, higher age and female sex may be factors that alone or in combination affect the increased need for blood transfusion/volume replacement in the perioperative period.
