Supracondylar Femoral Fractures Treated with Open Reduction and Plate Fixation

This study presents the results of surgical treatment of supracondylar fractures of the femur with use of open reduction and plate fixation. Patients operated from 2000 to 2003 we evaluated. From 58 patients and 61 fractures of all A-O types, there were 42 patients (45 fractures) avaliable for follow up. There were 21 males and 21 females, with average age of 52 years (19-91). 23 fractures were caused by traffic accidents (51%), 19 (42%) by fall, 1 by parachute jumping, 1 by press and 1 was patological fracture. 18 patients had only the left leg injured, 21 had only the right leg injured and 3 had the both legs injured. There were 9 open fractures (20%). Results. The patients were followed for a mean of 4 years 8 months (3-7 years). All patients were treated with internal reduction and plate fixation. 12 patients had complications that required reoperation. The cause for reoperation was refracture in 7 patients, infection in 3 patients and nonunion in 2 patients. The average time from injury to treatment for all fractures was 4, 5 days (0-43), but open fracture group needed more time to treatment (average 12 days). Conclusion. Open reduction and plate fixation is an effective technique for the treatment of supracondylar femoral fractures. Comparing our results with the results of studies about osteosynthesis with intramedullary fixation, we can conclude that intramedullary fixation gives maybe better results, but condylar plate is still reliable method for treatment of the supracondylar femoral fractures.
