A Prospective Study of ST-segment Depression in the Electrocardiogram During a 13-year Period

A prospective study of changes in the electrocardiogram was performed, analysed according to the Minnesota code in relation to ST- segment depression in a sample of 2414 subjects ; 1326 females and 1088 males, aged 35 to 54 years, chosen at random. Changes in the electrocardiogram were analysed on three occasions: 1969, 1972 and 1982. In 1969 the prevalence of total ST- segment depression amounted to 7.74%, 10.7% in females and 4.1% in males in a sample of the population aged 35-54 years. After three years the prevalence of ST-segment depression increased and in the total sample aged from 38 to 57 years amounted to 9.56%, in females 12.67% and in males 6.25%. The three-years incidence of ST-segment depression in females amounted to 7.3% and in males 4.0%. After a period of 13 years, in the same sample of the population (aged from 48 to 67years) the prevalence of ST segment depression futher increased, and in females amounted to 19.0% and in males 12.1%. The incidence of ST-segment depression during 13 years amounted to 15.4% in females and 10.7% in males, i.e.an annual incidence in females of 1.18% and in males of 0.8%. According to the results of this investigation it can be conducted that a finding of ST-segment depression of the descending or horizontal type represents a significant risk for midle-aged persons, which is indicated by the correlation beetwen ST-segment depression with Q wave, T-wave inversion, high R-wave of the "left" type, ventricular impairment of stimulus conduction and atrial fibrillation or flutter.
