Current Management of Psoriasis in Crtoatia
New psoriasis therapies offer great hope for the people who suffer from psoriasis and other chronic inflammatory skin diseases. Psoriasis and psoriatic arthritis are the dis¬ ; ; eases that can be treated with biological therapy which targets either the interaction between an antigen-presenting cell and the T-cell, or cytokines such as tumor necrosis factor alpha (TNF- a) that are presumed to be involved in the pathogenesis of psoriasis vulgaris and psoriatic arthritis. Current biologic therapies now seem to hold promise of potentially excellent effect: etanercept ; efalizumab ; infliximab ; macrolides (tacrolimus, pimecrolimus, sirolimus). Existing therapies in Croatia are: phototherapy (PUVA or UVB) for 4 weeks and for 6 weeks therapy: retinoids (Neotigason® ; ; ) + PUVA (RePUVA), cyc¬ ; ; losporin, methotrexate (MTX), and in very rare circumstances systemic steroids. Rota¬ ; ; tional therapy is necessary. Topical therapy includes: keratolitics ; tar (antralin, dithranol 0.5-4%) ; topical corticosteroids ; analogs of D3 vitamnine (calcipotriol, tacalcitol) ; topical retinoids (tazaroten gel) ; naphthalanotherapy ; heliomarinotherapy and combination of all above. In Croatia we use all therapeutic possibilities as they are in Europe, except bio¬ ; ; logicals which will transform the way of treating moderate to severe psoriasis. We prefer combinational therapy and principles of rotational therapy.
