Immunophenotyping as useful diagnostic method for atopic dermatitis
Atopic dermatitis is a chronic pruritic inflammatory skin disorder that frequently occurs in patients with personal and family histories of allergic disease. The relationship between allergy and the pathogenesis of the skin lesions in AD is not clear. In AD a large pro¬ ; ; portion of skin infiltrating TH cells, express a TH-2 like phenotype. Atopic dermatitis (AD) has cellular immunohistochemical features similar to those of allergic contact dermatitis (ACD) and there is plenty of evidence for T-cell activation in this disease. Tissue sections from the skin of fifteen patients with active atopic dermati¬ ; ; tis and five healthy controls were investigated by immunohistochemical staining. Stan¬ ; ; dard avidin-biotin immunoperoxidase staining of paraphin-embeded 4 µ ; ; m skin sections with semiquantitative counting of cells labeled with anti-CD3, anti-CD8, anti-CD-20, anti¬ ; ; HLA DR (HLA human leukocyte antigen), and anti-immunoglobulin E (anti-IgE) primary antibodies, was done. The analysis of cells in the dermis and epidermis showed that CD4+ prevailed over CD8+ lymphocytes in the infiltrate of AD skin lesions. In compari¬ ; ; son with normal skin, in AD skin a higher intraepidermal expression of HLA-DR+ Langer¬ ; ; hans’ cell count, HLA-DR on dermal lymphocytes and epidermal IgE+ cells was found. Comparison of AD patient group and healthy subjects control group showed that two groups differed in the number of infiltrating cells expressing CD3, CD4, and HLA-DR cells surface markers in the dermis and epidermis, CD8 in the dermis and IgE+ cells in the epidermis. We concluded that immunophenotyping is a useful diagnostic method in AD patients.
