Flow cytometric quantification of lymphocyte subpopulations in middle ear effusion (MEE)

Increasing evidence has been accumulated in the last few years associating immunological mechanisms in the pathogenesis of otitis media with effusion (OME). Immunological processes were found to be involved in both of the previously accepted experimental pathogenic models (Eustachian tube obstruction, microbiologically induced) of OME, but recently published data have provided evidence for the third exclusively immunological model. Tympanocentesis has opened a novel area to be explored in conection with OME. We previously published (1) three MEE immunophenotypic profiles and presume that certain phenotipic markers could be of possible diagnostic significance. The present study was conducted to evaluate the role of T, B and NK cells in local immune response. In thirteen children with OME the surface expression of 1-3 surface antigens in MEE and peripheral blood (PB) was investigated by three color flow cytometry with CD3, CD4, CD8, CD16, CD19, CD20, CD25, CD31, CD38, CD56, CD71, HLA-DR mAbs. MEE contained more CD71+ and CD4+71+ cells in comparison with PB, indicating the state of local chronic infection. This state was connected by CD16+/CD56+ cells decrease (MEE=6±4 vs PB=14±4). Moreover there was a significant decrease in the percentage of CD38 (54±17) especially on B cells (CD20+CD38-=5±3) in MEE in comparison with PB (79±9 and 2±1, respectively). This phenomenon appears to be due to suppressor activity of activated CD8+HLA-DR+ cells that were also found to be elevated in MEE. Our findings suggest thet T, B and NK cells may maintain an inflammatory condition in the middle ear (inability of T cells to turn on B to mature into immunoglobulin-secreting CD38 plasma cells) and confirm the role of immunological mechanisms in the pathogenesis of OME.
