Bilateral intermaxillary vertical dimension loss: implant-prosthodontic treatment options.

In extreme cases of intermaxillary vertical dimension loss, due to severe wear and/or caries, occlusal contact plane is developed between maxillary and mandibular edentulous ridges or residual roots. Implant-prosthodontic treatments of two cases of bilateral intermaxillary vertical dimesion loss, with adequate vertical dimensions in intercanine segment, are shown. In both cases it was decided to keep the present intermaxillary angle values, hence gaining the needed space by bilateral alveolar ridge reductions. Following endodontic and conservative treatment of remaining frontal upper and lower teeth, intermaxillary dimensions were recorded, OPGs were made, and vertical dimensional alveolar ridge measurements were made of lower jaws using specifically designed software. In both cases, the measurements showed that surgical reduction of approximately 4-9 mm bilaterally would ensure sufficient intermaxillary space for implant-supported fixed prosthodontic dentures without interfering with the intermaxillary angle. In the first case it was decided to perform a one-step surgery, during which the residual roots were removed, alveolar ridge reduced, and implants placed. In the second case the implant placement was delayed for 2 months after roots removal and ridge reduction. In both cases Ankylos implant system was used. Following the period of osseointegration in both cases fixed prosthodontic dentures were made, without changing the intermaxillary angle, therefore keeping the patients’ habitual occlusal relationships. Regular 6-months follow up has shown satisfactory results after 4 years.
