Carotid Arterial Disease in patients With Non-Insulin-Dependent Dibetes mellitus

Large population-based epidemiological studies have demonstrated that diabetes mellitus (DM) is a major risk factor for carotid arterial disease (CAD). The aim of this study was to examine the influence of non-insulin-dependent DM (type-2) to deveolopment of carotid atherosclerotic disease and to determine the influence of duration of DM on the carotid atherosclerotic changes. Our study group consisted of 60 patients (pts) with type-2 DM and control group of 60 non-diabetic pts. Color dopler flow imaging (CDFI) of carotid and vertebral arteries was performed in all subjects. In the group of pts with DM type-2 intima-media tickness (IMT) was found in 46.66 %, mild carotid stenosis in 28.33 % and moderate carotid stenosis in 18.33 % and occlusion in 1.66 % of cases. In control group intima-media tickness (IMT) was found in 50 %, mild carotid stenosis in 5 %, moderate carotid stenosis and occlusion was not found. Fisher test showed statistically significant difference between two groups (p<0.01). In the group of pts with DM duration under 10 yrs intima-media tickness (IMT) was found in 68%, mild carotid stenosis in 25% and moderate carotid stenosis in 7% of cases. In the group with duration of disease over 10 yrs we found IMT in 31%, mild stenosis in 35%, moderate stenosis in 31% and carotid occlusion in 3%. Spearman correlation showed statistically significant difference between two groups ( p<0.01). The study confirmed type 2 DM as important risk factor for CAD and showed that atherosclerotic changes of cerebral vasculature are intensifying with longer duration of disease.
