RELATIONSHIP BETWEEN HELICOBACTER PYLORI VIRULENCE PROTEINS STATUS AND HISTOLOGICAL PARAMETERS OF GASTRITIS

INTRODUCTION: The polymorphism of the clinical manifestations associated with Helicobacter pylori infection is potentionaly due to difference in virulence among individual Helicobacter pylori strains. There are no reliable virulence markers that can be used to predict the severity of the gastritis parameters of Helicobacter pylori infection. AIMS & METHODS: Aim of the study was to investigate the relation between the presence of antibodies to 12 Helicobacter pylori virulence proteins and histological parameters of gastritis. Study was done on 207 Helicobacter pylori positive patients undergoing upper endoscopy because of dyspeptic complaints. In all patients serum antibodies to p 120 (CagA), p 95 (VacA), p 67 (FSH), p 66 (UreB), p 57 (HSP homolog), p 54 (flagelin), p 33, p 30 (OMP), 29 (UreA), p 26, p 19, and p 17 virulence proteins were assessed using Western blot method. Histopathology of gastric mucosa was assessed using 2 antral and 2 corpal bioptic specimens and histopathological features were recorded and graded according updated Sydney classification. RESULTS: P 120 (CagA) positivity was associated with presence of higher grades of inflammation in corpus (X2 6.571, p = 0.087), activity (X2 11.15, p = 0.025) and intestinal metaplasia (X2 5.951, p = 0.114) in antrum of gastric mucosa. P 30 (OMP) positivity was associated with presence of higher grades of inflammation in corpus (X2 11.6, 4, p < 0.01) and antrum (X2 9.63, p = 0.025). P 33 positivity was associated with presence of higher grades of inflammation in corpus (X2 8.23, p < 0.05). p 19 (OMP) positivity was associated with presence of lower grades of intestinal metaplasia in corpus (X2 9.35, p < 0.025) of gastric mucosa. Different histological parameters had almost identical distributions despite serological status of bacterial proteins p 95 (VacA), p 67 (FSH), p 66 (UreB), p 57 (HSP homolog), p 54 (flagelin), 29 (UreA), p 26 and p 17. CONCLUSION: p 120 (CagA), p 30 (OMP), p 33 positivity and p 19 negativity was associated with presence of higher grades of histopathological parameters of gastritis. p 95 (VacA), p 67 (FSH), p 66 (UreB), p 57 (HSP homolog), p 54 (flagelin), 29 (UreA), p 26 and p 17 status didn't correlate with histological status of gastric mucosa.
