Retroperitoneal hemangiopericytoma with production of "big" IGF II involving hypoglycemia: case report

Hemangiopericytoma is a rare soft tissue neoplasm. Hypoglycemia has been reported in association with hemangiopericytoma and other mesenchymal tumors. In all such cases, blood glucose levels were restored to normal following complete or even partial tumor removal. It is postulated that an insulin-like growth factor family (IGF I, IGF II, IGF I receptor - IGF IR) might be responsible not only for hypoglycemia but also for malignant transformation. The level of IGF II in our patients sera was higher than normal and mostly in the large molecular form ("big" IGF II). Tumor tissue had IGF II, IGF IR and IGF BP1-3 mRNA transcripts. The tumor had multiple genetic alterations, including oncogene activation (overexpression and point mutation) and perhaps tumor-suppressor gene inactivation.
