Diagnostic value of contemporary myocard bioptate analysis on the diagram of diagnostic flow in primary cardiomyopathies in children
According to the literature, prevalence of mitral vale prolapse is about 10 % of population. Previous studies of patients with PMV have emphasized the frequent occurance of serious complications, new studies discuss about more benign nature. The aim of this retrospective study is to find prevalence of PMV in the population of cardiac ambulance patients during the period from 10/1999 - 02/2004, and to, using follow up study, find out the importance of prolapse with or without insufficiency. Among 1187 children, 688 had congenital hearth defect (57.9%), mitral valve prolapse was diagnosed in 51child (4.3%). In the group of patients with PMV there was 45 female(88.2%) and 6 male children (11.8%). In the time of diagnosis mean age was 12.42 2.91 god. (6.59-18.39 year). In 25 children prolapse was associated with mitral valve insufficiency (49.0%) and those patients endocarditis prophylaxis has been recomended. The time of follow up was 3.17 0.85 year (1.29-4.30 god.), or 161.78 years of follow up of all events in all patients. During follow up period there were no progression of insufficiency or other comlications. Subjective symptoms had 37 children (72, 5%), and 22 children were treated with propranolol (43.1%). Dolichostenomelic type of constitution was found in 10 children (19, 6%). Children with symptoms were more frequently treated with propranolol (p<0.01). There was no significant difference for grade of insufficiency for sex or constitution (p>0.05). PMV and SPMV are entity with good prognosis with need for endocarditis prophylaxis for group of patients with insufficiency, and children with profound symptoms should be treated medicamentously
