Background The concept of minimally invasive keyhole approach in neurosurgery has been increasingly used in the past few years. The approach is different from standard craniotomy because of smaller size of bone window, shorter skin incision, distinct microsurgical technique and instrumentation, and because it often uses endoscopic assistance. It implies small supraorbital craniotomy 2, 5 X 1, 5 cm with the skin incision in the eyebrow. Therefore, it can be considered to have certain advantages over standard craniotomy, particularly in terms of less brain exposure, shorter operation time, faster recovery, shorter hospital stay and excellent cosmetic results. Aims and methods: Between May 1996 and December 2004 a total of 782 patients were operated with this approach. Most of them were with vascular lesions (aneurysm), and 161 were with sellar region tumors. There were 58 pituitary adenomas , 50 meningeomas, 23 craniopharingeomas, 16 third ventricle extension tumors, 7 arachnoid cysts and 7 with other lesions. There were 90 (56%) females and 71 (44 %) males ; their age ranged from 18 to 75. The follow up period was from 6 months to 6 years. Preop. and postop. CT scans and MRI were performed, as well as other diagnostic methods.
