ENDOSCOPIC THIRD VENTRICULOSTOMY WITH ULTRASONIC CONTACT PROBE
Background: Authors present clinical application of the originally constructed ultrasonic contact microprobe (UCM) in endoscopic third ventriculostomy procedures. It is a new and effective tool in neurosurgical armamentarium as it enables safe fenestration of the third brain ventricle at minimal thermal and ultrastructural damage of the adjacent neural structures with maximal preservation of vascular structures. The newly designed UCM is 1.6 mm in diameter and easily passes through the 2.2 mm ventriculoscope working channel. Aim and methods: We analyzed a clinical course and radiographic data of 33 patients, operated by endoscopic third ventriculostomy with ultrasonic contact microprobe. Main indication for surgery was obstructive hypertensive hydrocephalus, mostly related to idiopathic occlusion of mesencephalic aqueduct. Results and conclusion: Results shows that endoscopic third ventriculostomy with ultrasonic contact microprobe is good alternative to the standard procedures of endoscopic third ventriculostomy with blunt probe or monopolar electrode in treatment of obstructive hydrocephalus.
