Impact of hyperglycemia on stroke mortality in diabetic and nondiabetic patients
We evaluate influence of stress hyperglycemia on stroke mortality in diabetic (DP) and nondiabetic patients (NDP). We included data of 630 stroke patients admitted at intensive care department within 24 hours of stroke onset. We correlate mean blood glucose levels (MBGL) at admission time and 72 hours after in DP and NDP (according to previous medical documentation) with their final outcome. Blood glucose levels higher tna 6, 7 mmol/L were treated as hyperglycemia. There were no statistically significant sex and age differences, therefor we have shown pooled patients data. There were 630 patients (mean age 71 ± 6) ; 410 were NDP (mortality 25% ) ; 220 patients were DP (mortality 20%). All patients who died within 28 days of hospitalisation had prolonged hyperglycemia (at admission and after 72 hours despite insulin therapy). Unadjusted relative risk of in-hospital mortality within 28 days in the group of NDP was 0, 68 (95% CI, 0, 14– 1, 9 ) and in the group of DP was 0, 39 (95% CI, 0, 22 – 1, 56 ). NDP with hyperglycemia had 1, 7 higher relative risk of in-hospital 28 days mortality than DP. Prolonged stress hyperglycemia in stroke patients increases risk of in-hospital 28 days mortality, especially in nondiabetic patients.
