Human brucellosis: an overview

Introduction: Brucellosis was totally unknown disease at our region Herzegovina until 2000. We recognized and described first cases and conclude that source of epidemic were donated animals. Humanitarian organization donated live stock for refugees and poor people. Objectives: To present clinical features of brucellosis in patient hospitalized at Clinical Hospital Mostar. Methods: 10 adult patients hospitalized in the department for infectious diseases were reviewed (May 2000.). Results: 9 man (manage 33, 2 years) and 1 woman (63 year) were recorded. All patients were in close contact with donated animals through their ordinary work. They were hospitalized from 14 to 46 days of illness with characteristic symptoms: high fever (100%), sweat (90%), headache (60%), fatigue (70%), neurological manifestation (5%), muscle pain (60%), back pain (70%), and loss of appetite (80%). Four patients had a local illness, 3 sacrolities and 1 had orchitis. Two patients suffered from diarrhea. The diagnosis of brucellosis was based on consistent clinical findings and a Wrights serum agglutination titer from 1/160 to 1/640. Brucella melitensis was isolated from hemocultures (2 patients). Seven patients were treated with doxycycline (200mg per day during 6 weeks) plus gentamicin (160 mg per day during 8 weeks) in the period from 2 to 6 month. All patients were completely healthy and there are no signs of relapse. Conclusions: Diagnosis of brucellosis in country without any experience with this kind of infection may be difficult. In our case fact about origin and distribution of donated live stock and epidemiological and clinical dates were very useful in process of making decision about epidemic.
