Background: The lipophylic yeast M.furfur is found as a part of the saprophytic microflora of normal human skin, particularly in regions well supplied with sebaceous glans. Several studies indicate that M.furfur plays an important role in the pathogenesis of atopic and seborrhoeic dermatitis. Objectives: The aim of the study was to determine the significance of M.furfur in the etiology of redness of atopic and seborrhoeic face skin lesions. Methods: The study comprised 50 patients with erythematous skin changes of the face with atopic dermatitis (AD) and 50 patients with seborrhoeic dermatitis (SD), as well as 30 healthy examinees without face skin lesions as a control group.The mycology analysis (microscopic examination andcultivation) on M.furfur was performed in all examinees. Skin prick tests and patch tests were carried out with water-soluble extract of M.furfur (protein concentration 5 mg/kg). Results: M.furfur was found by microscopic examination and cultivation in all our patients with AD and SD, and in 19 (63, 3%) of our healthy control subjects. Skin prick test to M.furfur was positive in 27(54%) of opur AD patients, 9(18%) of SD patients, and in only 1 (3, 3%) of healthy control subjects. Positive reactions in patch test to M.furfur were observed in 7 (14%) of AD patients, and in none of SD patients and healthy controls. It was statistically proved that positive reactions to skin prick test were significantly more often in Ad patients group than in other two group of examinees, and the same can be concluded for patch test results. Conclusion: The present study proved the role ofr immediate and contact hypersensitivity to M.furfur in exacerbations of AD, but not in SD. These findings enrich the diagnostic and therapeutic approach in AD patients, and may contribute to achievement of longer remissions of the disease.
