Prognosis of response to endoscopic sinus surgery in patients with chronic rhinosinusitis

The purpose of the study was to assess the prognostic value of some histopathologic mucosal findings of response to endoscopic sinus surgery in patients with chronic rhinosinusitis. Fifty (28M, 22F) patients with chronic rhinosinusitis underwent endoscopic sinus surgery. Patients completed a questionnaire concerning symptoms (congestion, postnasal drip, nasal secretion, headache, sneezing, cough, facial swelling and olfaction) before intervention and at 0, 12 and 24 months postoperatively. Biopsy specimens of the uncinate processes were stained with hematoxylin-eosin and examined histologically. The following parameters were assessed: edema, goblet cells, mast cells and eosinophils. Semiquantitative grading was performed in comparison with normal sinus mucosal histology (0, 1+, 2+, 3+). A correlation was sought between the histologic mucosal changes and symptoms preoperatively and during the follow-up. Significant improvement was noted for overall sinusitis symptoms score (p<0.05) during the whole follow-up. We found mostly positive correlation when comparing some histology with some patient symptoms (p=0.05 or p=0.01), both preoperatively and postoperatively. There were some differences in some histology and some symptoms between allergic and nonallergic patients. The data suggest that some histopathologic findings of sinonasal mucosa significantly correlate with some symptoms of chronic rhinosinusitis. Degree of histopathologic changes of sinonasal mucosa might be a predictor of response to endoscopic sinus surgery.
