Preventive effect of four glass-ionomer cements on hard dental tissues
Objectives: New formulations of glass-ionomer cements with higher ability of fluoride release have high significance in secondary caries prevention. Improved manipulating techniques as application of heat during cement setting are claimed to increase preventive efficacy of these materials. Aim of this study was to evaluate preventive effect of 4 glass-ionomer cements on demineralization of surrounding hard dental tissues (enamel and cement). Methods: 40 standardized class V cavities were prepared on buccal and lingual surface of 20 extracted caries free premolars with one half in enamel and one half in cement. 10 cavities of each group were restored with glass ionomer cements Fuji IX (GC), Ketac Molar (3M ESPE), Fuji 7 (GC) and Megacem (Megadenta) according to manufacturers instructions. Teeth were thermocycled for 1800 cycles between 5-55 C with dwelling time of 10 s. Following thermocycling teeth were immersed in demineralising solution (pH 4, 95) for 4 weeks. After 4 weeks teeth were rinsed, enrobed in epoxy resin (Citofix Kit, Struers) and sectioned in 2-3 sections that were evaluated under polarized microscope (100x magnification) (Opton) and photographed (Camedia, Olympus). Demineralisation of enamel and cement in contact with glass-ionomers were measured (Olympus DP soft, Version 3, 2) and statistical analysis performed using one-factor ANOVA and Tukey HSD test. Results: Showed lowest level of demineralisation of enamel in contact with Ketac Molar (517, 9 μ m) but significant difference between materials were not observed. On cement tissue significantly lower level of demineralisation was observed using Ketac Molar (578, 1 μ m) than Fuji 7(p=0, 004) and Megacem (p=0, 00003). Significantly lower demineralisation of enamel tissue was observed considering all tested materials (p=0, 00000). Conclusion: Hard dental tissues show lowest level of demineralisation in contact with Ketac Molar restorations. Application of heat during setting time does not improve preventive effect on demineralisation of hard dental tissues (enamel and cement).
