Does vertebral artery hypoplasia influence migraine aura
The aim of this study was to determine the effect of vertebral artery (VA) hypoplasia in the pathogenesis of migraine. We studied the extracranial part of the VA in 59 migraine patients (17 with and 42 withot aura) using color Doppler. In migraine with aura, five out of 17 (29 percent) patients had hypoplastic VA, and in migraine wihtout aura, 3 out of 42 (7 percent). The difference is statistically significant (p less than 0.05). In migraine with aura the mean diameter of the right VA was 2.7 +/-0.7 mm, and of the left 3.3 +/-0.7 mm. In migraine without aura, the mean VA diameter was 3.1 +/-0.5 mm on the right and 3.3 +/-0.6 mm on the left. In migraine with aura, the right VA was thinner than in migraine without aura (p less than 0.05). In migraine with aura, the mean systolic blood flow velocitiy was 55 +/- 16 cm/s on the right and 60 +/-17 cm/s on the left. In migraine wihout aura, the mean systolic blood flow velocity was 57 +/- 18 cm/s on the right and 57 +/- 18 cm/s on the left. In migraine with aura, we observed a higher frequency of hypoplastic VA, and a thinner right VA. Such findings suggest that hyoplasia of VA may contribute to the mechanism of hypoperfusion in the posterior circulation during the aura phase.
