Measurement of active movements by a bowley gauge and electronic axiography in persons with symptoms of temporomandibular disorders electronic axiography in persons with symptoms of temporomandibular disorders

The purpose of the study was to measure active mandibular movements in two groups of men by various procedures and to determine their significance with respect to the presence of symptoms of temporomandibular disorders. In the first group of men (N=190, average age 20) active movements were measured in millimetres by means of a Bowley gauge and marks on incisors. In the second group of men (N=31, average age 35) active movements were measured by means of electronic axiography (GAMMA CADIAX system). Both groups comprised subjects with and without symptoms of temporomandibular disorders. Data analysis was performed by programmes STATISTICA for Windows Release 5.5 H ('99 Edition) and SPSS for Windows, Release 7.5. The following average active movements were measured in the first group of subjects: maximum opening 50.0 mm, left laterotrusion 8.8 mm, right laterotrusion 8.2 mm, protrusion 4.8 mm. Symptoms of temporomandibular disorders were found in 46 (24.2%) subjects, but without statistically significant difference (Spearman's rank correlation) in relation to the ones without symptoms. Symmetry in performance of laterotrusive movements was established. The following average dislocation of the condyle in the temporomandibular joint and of the mandibular pathway was found: maximum opening right 15.087 mm and left 16.499 mm, left laterotrusion in the left joint 11.009 mm and in the right joint 3.136 mm. Right laterotrusion in the right joint amounted to 10.021 mm and in the left joint 2.746 mm. Right protrusion amounted to 10.264 mm and left protrusion 16.499. Symptoms of temporomandibular disorders were found in 11 (35.5%) subjects. By means of two-factorial variance analysis (ANOVA) of certain mandibular and temporomandibular joint movements no statistically significant differences were found among subgroups of the subjects. Measurement of active movements is performed during clinical functional examination. It is used as a screening test for establishing diagnosis of temporomandibular disorders with respect to pain and subjective sensation of limited movability of the patient's mandible. A descriptive analysis of records of certain mandibular and temporomandibular joint movements is a basis for a precise evaluation of the state of function of temporomandibular joints, but axiographic records are not a sufficiently reliable indicator of the state of function of temporomandibular joints. Measurement of mandibular movements and electronic axiography are considered as fundamental clinical diagnostic methods for temporomandibular disorders.
