Liječenje pretkliničkih neoplastičkih promjena vrata maternice dijatermijskom ekscizijom (LETZ)

In the period from December 1 1994 to January 31 1996 LETZ (loop excision of transformation zone) in 65 patients with preclinical neoplastic changes of uterine cervix was performed. All of them previously underwent cytologic, colposcopic and punch biopsy treatment. Vaginal and cervical inflammation was cured prior to definitive treatment. The highest age percentage of patients belong to the age group of 20-40 years (89, 2%). High grade intraepithelial lesions in this group was present in 43 patients (74, 1%) and 45 (69, 2%) in the total population. Bleeding after loop excision was teated by Semm´s cold coagulation in 6 (9, 2%) patients. It resulted in satisfactory healing, cytologic, colposcopic and clinical normalisation. Low abdomen cramp were treated as inflammatory reaction and ordered bed rest with consequent antibiotic therapy. Loop excision of transformation zone has its role in the treatment of preclinical neoplastic changes of uterine cervix, especially in younger population according to the accepted protocol. Its advantages are simplicity to perform, short term hospitalization, even ambulatory treatment, complete anatomical reconstruction of uterine cervix and its normal function in pregnancy. The main disadvantage of the loop excision technique is that it can be performed in a simple way, therefore we must resist temptation to use it too often.
